Do not use these forms to report incidents, events, or individuals that pose an immediate threat to life, property, or the Western community. If you require emergency assistance, call 911.

These forms may be completed by anyone, including students, staff, faculty, and community members. Forms may be submitted anonymously, if desired. Please keep in mind that federal/state regulations and University policy prohibit the sharing of most personally-identifying information and that providing your name/contact information as reporter does not guarantee that you will be contact or receive updates about the situation that you are reporting.

CARE Reporting Form

Report concerning behavior by a Western student, staff, or faculty member.

Bias Reporting Form

Report an incident of bias related to race, ethnicity, religion, age, socioeconomic status, or other.

Title IX Misconduct Reporting Form

Report an incident of harassment or misconduct related to sex.

Academic Concern Reporting Form

Report a student who is experiencing academic concerns.
