CARE & Concern Reporting
Do not use these forms to report situations or people that pose an immediate threat to themselves or others.
If this is an emergency, call 911.
These forms may be completed by anyone, including students, staff, faculty, and community members. Forms may be submitted anonymously, if desired.
This form is not monitored 24/7, so should not be used for urgent or emergency situations. Submissions will be reviewed during University operating hours.
Federal/state regulations and University policy prohibit the University from sharing most personally-identifying information. Providing your name or contact information as a reporter does not guarantee that you will be contacted or receive updates about the situation you are reporting.
CARE Report
Report concerning behavior by a Western student or employee.
Title IX Misconduct Report
Report an incident of harassment or misconduct related to sex, gender, or dating/relationships.
Notice of Discrimination, Harassment, or Retaliation
Submit a complaint of discrimination or retaliation in the workplace.
Academic Concern Report
Report a student who is experiencing academic concerns.
Pregnant & Parenting Service Request
Request information about pregnancy and parenting services for Western students and employees.
Ombudsperson
As part of Western’s Conflict Resolution Policy, found in Section 2.09 of the Handbook for Professional Personnel, faculty and staff may choose to engage with an Ombudsperson who can offer impartial and informal counsel for faculty and staff regarding workplace concerns, including issues of potentially unethical or illegal behavior.
